"As an oncology pharmacist with over 6 years of experience developing and implementing clinical pathways tools, I’ve seen firsthand how powerful these tools can be in improving patient outcomes, streamlining treatment decisions, and supporting the overall care process. But I’ve also experienced the challenges that come with integrating them into practice. Whether it’s overcoming resistance to change, making sure we’re incorporating the best available evidence, or keeping pathways updated and relevant, it’s an ongoing effort. Working through these hurdles is crucial if we want to fully realize the benefits clinical pathways can offer. The Journal of Clinical Pathways has been an invaluable resource throughout my career. It helps me stay up to date on the latest research and best practices and provides a space to connect with other healthcare professionals who are navigating many of the same challenges. It’s also given me opportunities to contribute my own knowledge, especially around oncology clinical pathway optimization, and receive meaningful feedback that’s helped shape my approach. This kind of exchange has been key to my growth as a pharmacist. The #340B Program was designed to help safety-net institutions—but is it inadvertently reshaping our healthcare landscape? As Deborah Williams and Anthony DiGiorgio, DO, MHA argue, the program’s original intent has morphed into an incentive structure that shifts oncology practices toward higher-cost hospital settings, crowds out private payer discounts and rebates, and raises healthcare costs for private payers.
