After losing his mother, Tyrone became homeless, overwhelmed by grief. His health declined, and after being hospitalized with congestive heart failure, he knew he needed a change. That’s when he was referred to Recuperative Care at our Long Beach site. With the help of Paul, his Housing Navigator, and Angela, his Enhanced Care Manager, Tyrone worked through his challenges. Though hesitant at first, he eventually embraced housing support. Together, they secured him a studio apartment, with move-in costs and medically tailored meals covered through his insurance. Tyrone will continue receiving Housing Tenancy & Sustaining services as he settles into his new home. His story is a testament to the power of integrated health and housing solutions—and the life-changing impact of compassionate care. ? We’re honored to share the inspiring story of Angeleen and John—a couple whose bond has endured over five decades through life’s most challenging moments. John discovered faith and sobriety during his 23-year prison sentence, while Angeleen faced addiction, homelessness, and significant health challenges. Angeleen found a fresh start at Whole Person Care Clinic's Recuperative Care program, where she began healing and regaining hope. After years apart, they fatefully reunited in our Imperial Valley program when John was referred to Recuperative Care. Their reunion after 23 years was a moment of pure joy, and since then, they’ve supported each other on their journey. Their story exemplifies how a stable and empowering environment can lead individuals one step closer to regaining stability and truly thrive in their communities. ?? After facing homelessness and immense personal challenges, Joey found hope through our integrated programs like Day Habilitation, ECM services, case management, job readiness, financial budgeting, and healthy living skills. With the help of his Housing Navigator, Gilbert, Joey not only secured a home of his own but also built a solid foundation for a brighter future. Gilbert shares: "Joey consistently goes above and beyond, exceeding every goal we set together. It’s a privilege to work with someone so driven, compassionate, and uplifting."
