Janet Belles and Mark Maitland, a nurse at ChristianaCare, forged a special bond after Janet’s car accident in September 2023. Maitland had been driving in North Wilmington when he came upon Belles who was trapped inside her burning vehicle. He rushed to help pull her to safety. The two would late reencounter each other at Christiana Hospital when Maitland happened to come into her room to put in an IV. They would reunite a third time as part of the Thank You Project, which enables patients and their families to express appreciation to caregivers who made a difference during their hospital stay. From elementary to high schools, students attending 25+ Delaware schools have access to ChristianaCare’s school-based health centers. Led by an advanced practice registered nurse (APRN), the team, composed of medical, mental health and nutrition experts, provides essential care to students at a convenient location. Three new elementary school-based health centers opened during FY24 to provide convenient, needed care to students right on site.

Since its launch in 2021, the ChristianaCare Hospital Care at Home program has become a national model for bringing acute care to the place most familiar to patients -- their home. The program’s multidisciplinary team, which includes nurse practitioners and registered nurses, provides the highest level of in-home acute care available statewide. With the use of technology and a collaborative team approach, nurses are able to monitor patients and provide a care plan suitable for their home environment. https://lnkd.in/e4KFpW4P

Oh what a day! Today was the Innovation Conference aka finale for the year long #AACN CSI HWE Delaware cohort! It was inspiring to hear all of the work that the 13 teams presented!! So many takeaways, great outcomes, and learnings from each presentation. Thank you #Bayhealth, #ChristianaCare, and #Nemours nurse leaders for supporting your teams in this endeavor! Thursday, 14 direct care nurse teams from Nemours Children’s Hospital, Bayhealth and ChristianaCare in Wilmington DE who participated in AACN CSI Academy shared their innovative project outcomes to improve the health of the work environment. Each team focused a project on one of #AACN healthy work environment standards. More than half of the teams not only improved the HWE standard their project was focused on, but all standards. Nurses leading change!!!!
