A Cessna 172 and a Diamond DA20 were involved in a serious airprox incident in the traffic pattern of Reykjavik Airport, Iceland. The minimum horizontal separation was 0 meters, with 75 feet (23 meters) between them. There was significant air traffic at Reykjavik Airport at the time of the incident. Yet, a television was on in the control tower, showing a football match in England. All controllers were attentive to the football match shortly before and when the incident occurred. Additionally, there was a disturbance at about the time of the incident after a controller had dropped a flight progress strip on the floor. There were no written rules regarding the use of the television in the control tower when the incident occurred. Following this serious incident, the television was disconnected.

The Kazakh Ministry Transport published a preliminary report on the December 25 crash of an Azerbaijan Airlines ERJ-190 at Aktau. The preliminary report states a sound was heard on the CVR at 05:13:31 UTC as the ERJ-190 had climbed through 3500 ft after being unable to land at Grozny. FDR parameters show continuous changes in pitch (between about +20° and -20°) and roll (between about +40° and -40°) for the final 70 minutes of the flight. The preliminary report also states numerous through and non-through damages of various sizes and shapes were found in the tail section, fin, stabilizer, and rudders. Similar damages were found on the left engine and left wing of the aircraft. The investigation commission extracted a large number foreign objects from the damaged areas, and sent them for additional forensic examination. > Download report: https://lnkd.in/eGGR28pK (PDF, in Russian)

A Cessna 550 Citation II was departing Barranquilla Airport, Colombia when the front right cargo bay door opened, causing items to be ingested by engine no.2, according to a newly released preliminary report. The investigation into this serious incident, which occurred on 18 May 2024, is continuing with a focus on the latching assembly and maintenance carried out. https://lnkd.in/ecfcJSpN

Azerbaijan Airlines flight J28243, an Embraer ERJ-190AR, crashed at Aktau Airport, Kazakhstan, after having declared an emergency. Flightradar24 data show the aircraft having fluctuating altitude and speed data. There were five crew and 67 passengers onboard. A fatal crash of a Sikorsky S-58T helicopter in South Korea in November 2022 was caused by a loss of tail rotor function according to a newly released accident report. The helicopter, engaged in a forest fire monitoring flight, entered a spiralling descent and hit a hillside in Gangwon Province, South Korea, killing all five on board . The Austrian Safety investigation authority published their preliminary report into the June 2024 hailstorm encounter of an Airbus A320 over Austria. The crew stated they had not seen anything on their weather radar. During the incident, two hail suppression aircraft were operating below the updraft area of the thunderstorm cell, introducing silver iodide, dissolved in acetone, into the cell. Whether the silver iodide introduced into the thunderstorm cell affected the reflectivity relevant to the weather radar remains under investigation. More info: https://lnkd.in/ef4e2-ce

The pilot of a Spanish Eurocopter AS 355N Ecureuil 2 was "showboating" at low altitude instead of carrying out traffic surveillance, when control was lost and the helicopter crashed. The newly released investigation report on the March 2023 accident also commented on the safety culture at the the DGT's aerial resources unit. https://lnkd.in/ek957uXF
