Are you developing large molecules that bind to their target in a specific organ? New research by Tatiana Zasedateleva, Stephan Schaller, Elizabeth (Liesbeth) De Lange and Wilbert de Witte show how binding to the target can cause depletion of the drug from that organ, and why this needs to be addressed across all stages of drug discovery and development! A new study by Shakir Atoyebi et al. reports a pregnancy PBPK model of drug–drug interactions between ritonavir-boosted atazanavir and rifampicin. The model suggests that ATV/r 300/100?mg BD could maintain antiviral efficacy when co-administered with rifampicin 600?mg OD in pregnancy. Can physiologically-based pharmacokinetic (PBPK) modelling be a tool for drug development with aldehyde oxidase (AO) substrates? Nihan Izat et al. present a PBPK framework for AO and dual AO-CYP substrates, emphasizing a totality-of-evidence approach to address challenges in translating AO-mediated metabolism. Realizing the full potential of clinical trials requires going beyond randomization: causal inference, and pharmacometric modeling and simulation help. In this article, Christian Bartels et al. show that by advancing both, non-linear mixed effects modeling implements the equivalent of standardization in causal inference.
