Trustworthy Artificial Intelligence for Personalised Risk Assessment in Chronic Heart Failure

The ambitious objective of the AI4HF project is to co-design, develop, evaluate and exploit an integrative and trustworthy artificial intelligence (AI) solution for personalised HF risk assessment, trained from large, complementary multi-source cardiovascular data, including (1) cardiac imaging, (2) cardiac biomarkers, (2) electrocardiography (ECG) and (4) information from clinical reports. The AI4HF technology will enable to dissect the precise characteristics of each HF patient, then predict the patient’s likely outcomes at an early stage to enable timely and personalised care. The project will put great emphasis on the concepts of trustworthy and inter-disciplinary AI, to ensure that the developed AI technology is trusted, approved and deployed in the real world for the benefit of patients, cardiologists, physicians, care providers and society at large. ????? Tech alone will not allow for a fairer society, we need to bridge the gaps in inclusivity and equality, namely gender gaps - and #AI brings an additional challenge! ?? Inequalities are rising in the OECD area for the last 4 decades and intergenerational mobility is no longer seen as possible ?? No young person should be forced to leave their country for lack of opportunities - societies that provide opportunities for their youth are more resilient at OECD And this last idea comes from the Portuguese Minister of Youth and Modernisation of Portugal Margarida Balseiro Lopes, one of the events co-organiser, together with the Ministry of Labour and Social Economy of Spain led by Yolanda Díaz Perez, and in partnership with AARP and Funda??o Calouste Gulbenkian.     Last week, I had the privilege of attending the Biomedical Linked Annotation Hackathon 9 in Tachikawa, Tokyo. The workshop revolved around a key theme: “Ensuring Robustness in LLM-based Research: Reproducibility, Interoperability, and Reliable Evaluation.” It was an incredible opportunity to advance the project "MultiSynDS: Large-Scale Multilingual Synthetic Discharge Summary Generation Using Generative LLMs and Clinical Case Report Transformation." Our primary focus was leveraging an LLM-as-a-judge approach to assess the quality of discharge summaries generated by Large Language Models in the Cardiology domain. We also explored how these evaluations correlate with expert annotations provided by Cardiologist Dr. Leticia Fernández López. This dataset will be of key interest for tasks such as Data Augmentation for clinical NLP systems training, providing them with data samples similar to real life reports created in clinical sites such as hospitals. The organization of the Hackathon was exceptional, thanks to the efforts of the organizers and our excellent hosts at Research Organization of Information and Systems (ROIS-DS) and Database Center for Life Science (DBCLS). A special mention goes to Jindong Kim for his outstanding presentations and event management throughout the workshop. A big thank you to Saurabh N. for being an outstanding collaborator during the hackathon and helping achieve meaningful results. I’m also deeply grateful to Dr. Martin Krallinger and Dr. Carlos Escolano from Barcelona Supercomputing Center for their expert guidance and support throughout this ambitious project within the frameworks of DataTools4Heart and AI4HF (EU project).     ?? Over 50 active participants in the International Workshop Design and?Implementation of?Inclusive?Clinical?Studies, organised by SHINE 2Europe on 21st?January 2025.? ? The event explored this topic within 4 projects RadioVal, Liveration, AI4HF (EU project) and IHI READI, all in which clinical studies play a central role. ? Designing inclusive clinical studies is often challenging due to time, human, and financial constraints, which tend to overload the professionals developing them, but overlooking inclusivity poses an even greater risk: the exclusion of underrepresented groups from research and even from optimal medical care. During the workshop, a wide audience – including researchers, healthcare professionals, AI and IT developers, ethicists, regulators, policymakers, social scientists and patients – analised the main challenges to achieve more inclusive clinical studies, potential solutions and the relevant stakeholders involved in each. ? Four high-level speakers presented the featured projects, focusing on the clinical studies aspects, and providing insights into how aspects of diversity, equity and inclusion have been considered in the design and will be further addressed in the implementation of such studies. The group of participants was then divided into three break-out rooms to discuss in-depth the challenges for inclusive clinical studies, divided into different domains, such as ethical, legal, social, technical, organisational and others. Then, in each group, the challenges were prioritised and the one considered as most pressing was further analised in terms of potential solutions and the relevant actors that can enable such results. ? During the discussion, several challenges emerged, such as:? ?? Literacy, namely health, digital and traditional literacy ??Diversity, equity, and inclusion training for all stakeholders involved in the design and implementation of clinical studies, especially for clinical and research teams ??More flexibility in the application of a study protocol, to encourage inclusion? ??Access, on a geographical base, as trials are often conducted in urban or academic centres, limiting access for rural (or smaller towns) populations? ? A very urgent challenge identified was also the limited time and funding available for trials and clinical studies, which significantly impacts their implementation.? Proposed solutions included calling on the European Commission to design calls for proposals that better address the specific needs of trials, promoting greater collaboration to overcome organisational barriers and using innovative digital tools along with comprehensive routine data collection for patient selection, thereby ensuring greater equity.
