When I got this link from one of my friends, it looked like a phishing form. But then I found the same form on the OpenAI help webpage.

• Enter the email address associated with your ChatGPT account. ( login into your account at https://platform.openai.com/ )

• Find your Organization ID in your ChatGPT settings and enter it into the opt-out form.
• Enter your Organization Name found in your ChatGPT settings.

• Solve the Captcha to verify that you are not a robot.
• Submit the data opt-out form to OpenAI.
• Check your ChatGPT account for a copy of the form to confirm that the opt-out request was successful.
