Over the years shipping has identified a few critical operations which need our undivided attention in planning, preparation and execution stage for operations involving Enclosed space entry, Hot work outside safe area (ER Workshop), Working on heights (above 2 mtrs), any type of internal or external oil transfer.
Special attention should be given to operations simultaneously involving two or more of the above. Due to the severity of psychological and economic impact of any incidents arising from these operations, an effective ship management team will always take them seriously.
Due to the timing of these operations and the size of team which needs coordination to carry such jobs, they have become an easy target to find gaps in SMS implementation. Considering the above it's good to maintain the timeline as per actual execution of the job, to avoid a situation where an explanation can be demanded even after a safe operation.
A factual timeline on permits would help to pinpoint a root cause in case of undesired outcome, thus making the overall organization a safer, aware and effective place of work.
