I wold say that in my experience, there are two factors that need to precede the creation of this safe and healthy culture, which are more challenging to implement when you ahe an existing tem: 1) You must hire for cultural fit over skills - the former is innate, the latter can be learned/taught. 2) You must exist anyone from the business who doesn't fit this culture, as that one bad apple will totally spil the barrel of they remain. :-)
