Why Project RED (Re-Engineered Discharges)?

Errors happen when patients move from one healthcare setting to another including returning home. These "Transitions of care" are gaps in systems that can trigger inappropriate hospital readmissions, the need for corrective procedures, and other negative clinical and financial consequences.

Led by Dr. Brian Jack as the Principal Investigator, the revolutionary research published in the Annals of Internal Medicine, the 2009 article "A Reengineered Hospital Discharge Program to Decrease Rehospitalization", has subsequently been cited in almost 2,000 articles.

Since its initial publication, the thirteen essential principles established by Project RED have been applied to numerous healthcare settings - hospitals and clinics, long term care, behavioral health, and more - to dramatically improve clinical outcomes, medication compliance, as well as staff and patient satisfaction.

Under the direction of Dr. Jack, Project Red Solutions was created to offer consulting and training services to share knowledge and experience with healthcare providers, assessment & accreditation bodies, governments and other to utilize Project RED's best practices.

Improving Transitions of Care domestically is just as important for patients traveling away from home for healthcare - perhaps even more so. Closing gaps in the patient journey - logistical as well as clinical and behavioral - are more complicated for medical tourists. For this reason, we are part of the team at Project Red Solutions to encourage providers who care for health and medical travelers to create and deliver an improved discharge process.
