Partnerships That Increase Client Well Being

While we’re excited for all that this collaborative relationship will bring, we've begun seeing the undeniable value of what can happen when we take a holistic approach to our care plans. Just recently we had a client whose life was touched by the incredible expertise and services of Jukebox Health.

This client has been living at home full-time after being diagnosed with both Parkinson’s and Alzheimer’s. Although he lives with his wife and a team of full-time caregivers, it became clear that as his symptoms progressed, he would no longer be safe living at home.

For anyone who knows, lives with, or treats someone with a chronic condition you are aware of how challenging it can be to ensure one can maintain their safety, independence, and comfort. And unfortunately, many homes are not designed to cater to those with chronic conditions.

After the client began attempting to get out of bed on his own, his caregivers became concerned about the likelihood of a fall or injury in the middle of the night. Once Alliance heard about the situation, we referred the family to Jukebox Health. The Jukebox Health team got in contact with our Alliance caregiver, reviewed the initial report, and got to work immediately.

A Thorough Approach to Home Safety

To start, Jukebox Health sent a home safety expert, specifically an occupational therapist (OT), out to the home to conduct a thorough safety checkup. During this checkup, the OT accessed each room to pinpoint safety concerns.

In addition, the home safety expert worked directly with the client’s wife to better understand her medical history, fall history, medications, and other aspects of daily living that could increase the client's risk of an at-home fall.

After the evaluation was complete, it was discovered that besides nighttime wanderings, the client was experiencing difficulty with bed mobility. Jukebox Health offered the family several potential solutions. These included:

- A bed railing to limit nighttime impulsivity;

- A bed mat alarm to alert Alliance caregivers in the event that the client got out of bed on his own;

- And to improve bed mobility for morning routines with the caregiver, a bed ladder that the client could use to pull himself up from lying down to a seated position.

Jukebox Health was able to provide the products, schedule installation, and provide 1:1 support to our client until the project was completed.

Since the home modification project, the client has shown a significant decrease in nighttime wandering, and can safely complete transfers into and out of bed with assistance from an Alliance caregiver. This has provided more seamless access to other care tasks such as bathing, toileting, and dressing routines.
