Why Doesn’t Anyone Trust Doctors Anymore?

Confession time: This week, in light of RFK Jr.’s confirmation to lead the Department of Health and Human Services, I had planned to use this space to reflect on the health policy changes coming from the new administration and how our physician community is coping with them. But over the past several weeks, I’ve come to realize there’s a much larger story than RFK Jr., MAHA, or vaccines that poses an urgent and existential threat to our profession: the erosion of trust in doctors in America. It’s become clear to me that we as physicians and a healthcare system have lost the trust of the public. Consider this simple question: Why is it that Mr. Kennedy’s lack of credentials and expertise — and of basic knowledge about the agencies under his purview — are exactly what many people find so appealing about him? The erosion of trust in medical experts has been simmering for years, but spread like wildfire during Covid and will almost certainly now gain steam. And as measles spreads in Texas; Ebola spreads in Uganda; Tuberculosis in Kansas; Avian flu in Nevada; and Mpox in New York, I believe it’s urgent that we as physicians have an open conversation about what we can do to gain back the public’s trust.

So this morning, I published my own ideas about how we might go about doing this — based on lessons from my interactions with ER patients who once distrusted my advice, but ended up believing it. Could this work on a societal level? I suspect so. Lesson 2 "demonstrate transparency and humility" seems counter-intuitive but is one of the best ways to build trust. Sometimes it also requires acknowledging that there are multiple reasonable options and using shared decision-making to select the best one for the individual case.
