Why Do We Fail to Implement Safety Learnings
BP Texas City is one of the most studied industrial accidents in US history. There were significant learnings. The blast was caused by organizational and safety deficiencies at all levels of the organization. The investigation triggered new guidelines, new safety programs and more training. However, the industry’s serious injury and death toll has barely slowed. This isn’t just the refining industry it applies in construction and other industries as well.
We have known for decades how to prevent deaths in trenches however, workers are still killed in unshored or slopped excavations.
