Why accident Occurs?

During my entire career (almost ten years), I have come to one conclusion about accident occurrence at the workplace, 99.99 % it is due to human error and 0.01 % due to machine error.

WHAT IS HUMAN ERROR?

Negligence, No or Low concentration to hazards, No or low Maintenance, ignoring safety rules and standards e.g. work instructions, SOP (Standard Operating Procedure), No or less training (regarding safety standards as well as Work procedures), No competency, bored/ tired/ uninterested to job, No or less PPE (personal protective equipment) etc.

WHAT IS MACHINE ERROR?

A machine error is an error caused by a physical defect issue instead of an error caused by a human in the software code or by the user of the hardware device.

Sudden machine failure due to the following reasons: -

1.     Wear and tear

2.     Software failure

3.     Momentum loss

4.     Burst / Explode

5.     Heat up or tangled – Parts

6.     Breakage and more…

WHY ACCIDENT OCCURS AT WORKPLACE?

1.     Competency:

The workers or operators working inside the production zone have no competency, either they are lacking work experience or have no education (training) related to a specific job.

2.     Low or Zero supervision:

Human being by nature asks for relax environment, if no or low supervision, they ignored many safety rules and work instructions just for their ease.

3.     Attitude / Behaviours:

The way of performing a job can lead to an accident if they show aggressiveness or their behavior to do a job is not as per standard and they behave like they know everything.

4.     Instructions (safety and work)

Job steps for a particular activity are not clear and no safety knowledge (SOP, PPE, Signs, Posting, Training) is there.

WHAT IS HAZARD?

Anything that has the potential to harm human being, property, and environment is called hazard, from a simple pen nib to a complex machine, from sitting in the office under the ceiling to moving out in the market, almost everything is hazardous in the universe.

It is our way of action, the method we follow, the procedure we use that determines the degree of hazards, and it can be low, medium and high, as per our way, method, techniques, and procedure. Pen for writing is good and safe, but the same Pen for fighting is high hazardous (sharp nib), paper for writing is good and safe but it is highly flammable, sitting in the office is good and safe, but sitting for a long time can lead to ergonomic hazards….

HOW ONE CAN AVOID ACCIDENTS?

1.     Competency (Experience + Knowledge/Education/Training)

2.     Monitoring

3.     Safety Standards & Postings

4.     Work Instructions

5.     Revision of Procedure after a particular period.

6.     Training

7.     Job breaks

8.     Motivation (Positive culture promotion)

9.     Maintenance (All kinds)

10. Third-party certification if required etc….
