"The lens through which you look at a problem is probably the same lens through which you solve the problem." The question is, how many other lenses did you neglect in your solution?
I'm getting old. No matter that I exercise daily, eat well, and get enough sleep, things are starting to ache. No, this is not going to be an article of all my aches and pains, my wife hears that from me often enough.
This has to do with a recent physical therapy visit and what I witnessed that had so much overlap to continuous learning and performance improvement I had to write about it.
First - the Outcome. The initial consultation with the physical therapist, mirrored the performance outcome assessment phase in the continuous learning and performance improvement process. The PT asked probing questions about my medical history, lifestyle, and then he got into it.
"What do you want to get out of our work together?" This was an interesting first question because it set him up for the "but..." (to be explained later). My goal is to minimize, if not eliminate my pain, without surgery or medication.
Second - Analysis...Plus. The PT shares his understanding of my goal and then says, "What do you do for exercise? Describe your work routine. Tell me about your typical daily nutritional intake. Describe your down time and sleep habits."

It was evident that the PT, much like the HPT model, was trying to understand the gap between the desired outcome and the current state including all the impacts on the outcome.
We spend the next 30 minutes going through my typical exercises. I show him what I do and how I do it. He observes my form, he notes the weights I use, and the number of repetitions for each exercise. He finishes this by introducing me to some new exercises to strengthen the muscles near my shoulder.
Third - Setting Expectations. As I'm leaving, he hits me with "I'm going to draft an exercise plan for you which will be available in the app we discussed. When we meet next week, we will discuss the training plan, the milestones, how the program will grow if we hit our milestones, and adjustments we will make if we are not hitting them. "BUT, even with all this we may still get to a point where medication or surgery is necessary."
