Frequency is not adequate (insufficient refresher training):

Use technology, such as e-learning platforms, to facilitate continuous learning.

Inattention to details:

Foster a culture that emphasizes the importance of attention to detail.

Implement checklists to guide individuals through tasks.

Conduct periodic reviews to identify and correct lapses in attention.

Assess and address skill gaps through targeted training programs.

Provide mentorship or coaching for individuals who need additional support.

Consider hiring or reallocating resources based on skill requirements.

Addressing these root causes requires a holistic approach that combines improvements in documentation, training, and organizational culture. Regular evaluations and feedback mechanisms will help identify ongoing areas for improvement.
