There are some possible root causes for human error, which I have tried to explain.
The procedure is not clear.
Revise and improve written procedures to ensure clarity.
Use step-by-step guides with straightforward language.
Obtain feedback from users to identify and address areas of confusion.
Ambiguous or confusing instructions:
Review and rephrase instructions to eliminate ambiguity.
Seek input from end-users to identify and clarify confusing elements.
Use visual aids or diagrams to supplement the written instructions.
Lack of sufficient details:
Enhance the documentation by including comprehensive details.
Provide examples or case studies to illustrate key points.
Encourage feedback to identify and address gaps in information.
The document format is not adequate.
Design documentation in a clear and user-friendly format.
Use headings, bullet points, and visuals to enhance readability.
Ensure easy access to documents, and update them as needed.
Training instructors are not adequate.
Invest in training instructors' skills and knowledge.
Implement a feedback system to evaluate the effectiveness of training.

Insufficient practice or hands-on experience:
Incorporate practical exercises and simulations into training programs.
Encourage employees to participate in real-world scenarios.
Provide opportunities for regular skill reinforcement through practice.
Frequency is not adequate (insufficient refresher training):
Establish a schedule for regular refresher training sessions.
Integrate refresher modules into ongoing training programs.

Inattention to details:

Addressing these root causes requires a holistic approach that combines improvements in documentation, training, and organizational culture. Regular evaluations and feedback mechanisms will help identify ongoing areas for improvement.
