Simply put, FeldCare Direct links in-home outpatient rehab patient referrals to the OTs, PTs, and SLPs who can help them.?

Doctors, home health agencies, hospice agencies, discharge planners, rehab facilities, and more can refer patients to a therapist through the FeldCare network.?

FeldCare Direct Referrals

Here’s how it works when a physician, discharge planner, rehab center, or rehab specialist wants to refer to an OT, PT, or SLP through the network.

Step One: Call (818) 538-5880 to connect with the FeldCare team, or send the in-home outpatient referral via fax at (818) 616-6676, and we will check your patient’s eligibility.?

Step Two: Once billing knows a patient is eligible, the doctor’s order and consent form is obtained.?

Step Three: An OT, PT, or SLP utilizing the FeldCare Direct network will connect with the patient referral. From here, the therapist will begin the plan of care.?

Step Four: FeldCare Direct will submit claims to the insurance company, and once submitted, the clinicians will be paid for their services from those claims.?

Conditional: Sometimes, patients don’t have a prescription for physical therapy, but they would like to work with a PT for rehab or wellness. Direct access applies to this situation. They can call (818) 538-5880 to see what therapists are available in the network.

How Clinicians Connect with FeldCare

Clinicians who use the FeldCare network are all independent contractors working as freelancers. This makes using FeldCare a unique experience for those used to working in a clinical setting.?

Here are the steps clinicians can follow to begin connecting with patient referrals through FeldCare:

Step One: A clinician will connect with FeldCare through this page on our website or by emailing their contact information and CV to [email protected].?

Step Two: Someone from FeldCare will reach out and interview the clinician, verifying their information and sharing more about FeldCare.

Step Three: Once the clinician decides they would like to use the FeldCare network, they will submit all of their credentials and provide all necessary licensing and insurance information.

Step Four: The clinician will be briefed on how to utilize the network and more.

Step Five: Finally, they will gain access to the cliniCONNECTS app, which allows them to connect with local referrals. Here they will create their profile and begin to be notified about patients in their area. They can accept or reject patients based on their caseload, schedule, or any other reason.?
