1. Misinformation
When a doctor fails to inform the authorities and/or patient, he stands the chance of recording incorrectly
2. Repetitive Records
When bad or no records are kept, this can often lead to duplication of tests and other investigations - besides accruing double bills, it can be confusing when diagnosing
3. Lengthy hospital stays
If the differential diagnosis procedure isn't carried out correctly, it can lead to prolonged stays in the hospital or repeated visits - again, leads to unnecessary bills for the patient
4. Prolonged illness or worse
If records are incorrect or poorly kept, it can lead to a worsening of the patient's health and or death
