UX touch points in Scaled Agile

As a user experience (UX) professional who has been aware of Scaled Agile Framework (SAFe) since the 1.0 days, I've always thought that UX was under-represented (and not well understood) in the model. On one hand, SAFe does a great job showing how an enterprise can come together to support Agile development. On the other, it under-represents the UX and design touch points necessary to ensure quality user-centered products are rolling out the door.
In SAFe v4.5, we see a continuing evolution of how UX is included in the overall system, but when I speak with other Agilists, no one seems to understand the breadth of UX or when they should be considering its input across the system. Typically, I only see people asking for UX input within Agile development teams (team level), and trying to complete all the upper-level UX activities in a very compressed format. Even if there is a UXer at the ART, they are typically overtasked and under supported.
I took a pass at the v4.5 framework to identify UX touch points - areas where there are some important UX activities and work products that can be considered in user-oriented organizations. Not that this is comprehensive, but generally, if you are an organization with user-centered goals, the idea is, can you identify who and what is being done in these areas that ensures UX involvement?
