The Social Emergency Medicine Section at USF/TGH works to integrate structural competency into medical education and patient care through innovative assemblages of new pathways.
Our work with infectious disease and substance use disorder best exemplifies early success of the model.
Our pathways are built through collaboration with social scientists, public health experts, physicians, other care providers, and patients. We believe that direct clinical involvement of social scientists is a novel key to our success.

2015
USF EM (Jason Wilson, MD) initiated non-targeted HIV screening in the ED utilizing cheek swab rapid test kits administered by in-kind staff support from DOH contracted organizations. Testing volume was low (1-2 patients/day)
2016
Jason Wilson, MD, USF Emergency Medicine, was awarded a Frontline of Communities in the United States (FOCUS) award that funded serum based non-targeted (CDC guideline based) HIV and HCV screening in the ED.
2017
Expansion of the program to TGH Brandon Healthplex
Through IT support and automation, testing volume rose to, and stabilized at approximately 22% of the ED volume over the age of 13 (over 1,000 HIV and over 1,000 HCV tests per month).
ED based HCV screening demonstrated high volume of people who inject drugs (PWID) among new cases of HCV outside of age cohort (born after 1965)
2018
ED BASED MAT (ED BRIDGE)
Launch of the Medication Assisted Therapy (MAT) or Medication for Opioid Use Disorder (MOUD) in September 2018. ED MAT has been named ED BRIDGE (Building Recovery Integration for Drug Users)
Features of ED BRIDGE include buprenorphine in ED, buprenorphine bridging Rx, peers, and direct linkage to care
Partnerships for Warm Handoff Established with DACCO (now the Cove), ACTS, and collaboration with Central Florida Behavioral Health Network (CFBHN)
2019
Services were also expanded to maternal antepartum and postpartum patients
Jason Wilson, MD was awarded a NIDA ED-INNOVATION site award, providing multiple years of research assistant, Peer and medication in ED funding for Buprenorphine while measuring outcomes in patients given injectable buprenorphine compared to those that receive sublingual buprenorphine. Initiation of ED based take home Naloxone Nasal Spray [Narcan] program (funded through DCF/SAMHSA/SOAR), expanded to IDEA and hospital floor
ED BRIDGE program was expanded to USF Internal Medicine and the addition of the USF OBOT clinic
Addition of Peer Recovery Specialists: through SAMHSA/SOAR funding and partnership with Central Florida Behavior Healthcare Network, the first ED based certified peer recovery specialist was employed. In 2020, a second peer joined the ED team and, in 2021, we received funding for a third peer.
2020
ED BRIDGE expanded to include alcohol, benzodiazepenes and other substances beginning in 2019 and integrated further in 2020.
2021
IDEA Tampa
Infectious Disease Elimination Act (IDEA) expanded by Florida Legislature and signed into law by Governor Desantis on June 27, 2019 to allow any county to sanction one sterile syringe exchange program (aka syringe services program, SSP).
SSP has formal evaluation program operated jointly by USF Department of Anthropology and USF College of Public Health
IDEA Tampa is jointly operated by the Social Emergency Medicine Section within the Division of Emergency Medicine, Department of Internal Medicine, Morsani College of Medicine the USF Tampa Bay Street Medicine, Department of Internal Medicine.
Jason Wilson, MD and Asa Oxner serve as program Medical Directors.
The SSP is supported by grants and awards, many of which are tied to HIV and HCV testing and linkage to care
IDEA awarded FOCUS funding in December 2021 to expand phlebotomy training to conduct HCV RNA and other serum testing
IDEA Participants are provided Nasal Spray naloxone [Narcan] and postage paid postcards to anonymously report Narcan use for overdose reversal
Tampa General Hospital has an agreement with Hillsborough County Board of County Commissioners and letter of support from DOH to operate the SSP
IDEA Tampa Syringe Services Program (SSP) launched in January 2021.?The Florida DOH has more information about the Infectious Disease Elimination Act.
Over 2 million dollars of HIV, HCV, and opioid use disorder support has been provided through the FOCUS award between May 2016 and May 2021. In May 2021, USF EM/Tampa General Hospital entered grant year 6. Other financial support has come from the IDEA Miami program, the Comer Foundation, Avenir Award, and the Hillsborough County Ryan White EHE Program
2022
Jason Wilson, MD will serve as site PI for a PCORI funded project (AD-2019C1-16066, PI = Conner, USF) aimed at decreasing health disparities during transitions of care utilizing peer navigators.IDEA Tampa awarded Comer grant for supplies and associated SSP costs
Further integration of medical anthropologists into clinical pathway creation will also take place through exploration of the CMS Hospital at Home program which will require in-home social determinants of health assessments for success.

Social Emergency Medicine Section Publications
The USF EM SEM Section is exemplified well through work on opioid use disorder and infectious disease. However, we also have made an impact working with problems around low English proficiency, firearm violence, stigma and suffering of Sickle Cell Disease, patient experiences and mental illness. Much of that work is further outlined in a new book, Clinical Anthropology 2.0, as well as the publications below.

The FOCUS Program is a public health initiative that enables partners to develop and share best practices in routine blood-borne virus (HIV, HCV, HBV) screening,diagnosis, and linkage to care in accordance with screening guidelines promulgated by the U.S. Centers for Disease Control and Prevention (CDC), the U.S. Preventive Services Task Force (USPSTF), and state and local public health departments. FOCUS funding supports HIV, HCV, and HBV screening and linkage to the first medical appointment after diagnosis. FOCUS partners do not use FOCUS awards for activities beyond linkage to the first medical appointment.
