The Untapped Resource in Post-Acute Care

A recent study in the Journal of the American Geriatrics Society highlights that "over 30% of older adult caregivers report simultaneously receiving care for their own daily activities."?In other words, a lot of care is being delivered by family and friends.
Given the cost and limitations of a smooth and satisfying patient experience is it any wonder patients are turning to those they love and trust for help?
This may represent a powerful and cost-effective resource to leverage for care teams if this activity is coordinated with the care team's efforts, particularly those operating in value-based care models and in post-acute care.
Conversely, left uncoordinated this activity may be at cross-purposes with what the care team is trying to accomplish, and undermine those clinical efforts jeopardizing the outcomes so crucial for financial success.
How many organizations have recognized this untapped resource and have a strategy and infrastructure in place to leverage it for better outcomes?
Implementing purpose built tools for communicating not only with a diverse and dynamic care team supporting the patient, but able to include the patient themselves along with their family and friends is a great way to leverage this untapped resource. Tools like those offered by Hucu.ai are a great way to get started.
