Unmasking the unintentional gaslighting: the power struggle within hierarchy

Maintaining a strict hierarchy, concealing any errors even if inadvertently made, and avoiding any display of vulnerability are essential. I don't believe that the gaslighting that occurs in such situations is necessarily intentional. Some have argued that what I describe may not fit the traditional definition of gaslighting due to its lack of intent. However, I contend that it can be a learned behavior, a survival mechanism ingrained over time. It's akin to strategies we employ in the wild – a 'dog-eat-dog' world where posturing and undermining others' authority become tools to safeguard our own position within the system.

Master stress with Navy Seals' secret: The 4-4-4-4 box breathing technique

I delved into the concept of distress tolerance, particularly focusing on practical skills in this area. A prime example is the box breathing technique, famously used by the Navy Seals, known as the 4-4-4-4 method. This simple yet effective method involves a cycle of inhaling for 4 seconds, holding the breath for 4 seconds, exhaling for 4 seconds, and then pausing for 4 seconds before the next inhalation. What sets this pattern apart from other breathing techniques, such as the 5-6-8 sequence often found in meditation practices, is its non-physiological nature. This unique aspect demands a higher level of concentration, which can be instrumental in resetting the vagal nervous system, making it a straightforward yet powerful tool for managing stress.

Empathy in addiction recovery

A mother and her son arrived at the office one day, unaware that their physician had decided to close the doors without informing them. The abrupt closure left them in a state of panic, as the medications required for their medication-assisted therapy needed to be tapered gradually; abruptly discontinuing them was not an option.

Their panic was palpable as they grappled with the uncertainty of their situation. Desperate for help, they reached out to our office. Fortunately, we managed to schedule them for a same-day appointment, marking the beginning of a meaningful relationship.

During one of our conversations, I expressed how honored I felt to be part of her care. To my surprise, she responded by saying that nobody had ever said that to her before. This encounter highlighted a recurring theme in the addiction community – a history of negative experiences. Many individuals in this community have faced ostracism, humiliation, and agonizingly long waits to access medical care or treatment. Her words resonated deeply with me, emphasizing the importance of empathy and compassion in our work.

Preparing to enter a patient's room for a challenging encounter, or gearing up for a complex procedure, I'd often pause just before crossing the threshold, taking a few deep breaths. As unremarkable as it may seem, this simple practice had a profound impact on me. Revealing the power of error disclosure

I'm baffled by the occurrence of errors being sometimes concealed or ignored, as this practice ultimately benefits no one. From my perspective, errors should not be pinned solely on any one individual, whether they are intentional or unintentional, as they often highlight weaknesses within existing systems. When errors do arise, patients have three fundamental expectations:

Firstly, they expect full disclosure – a clear understanding of what transpired and why.

Secondly, they anticipate concrete steps taken to prevent a recurrence of the same issue.

Lastly, they yearn for a sincere apology.

Embracing these principles and actively listening to our patients can lead to benefits for all parties involved.
