Unfolding the Thalidomide Story: The Creation of a Public Health Comic Book

At Transmissible, we believe in the power of storytelling to convey complex public health lessons in an impactful, memorable way. Our latest project, "The Thalidomide Case: From Molecule to Catastrophe," vividly illustrates this approach. Through visual storytelling, we highlight one of the most tragic chapters in pharmaceutical history and use it to educate the next generation of pharmaceutical and public health professionals. This book is part of the Erasmus+ project "From Molecule to Drug: A Universal Necessity" (2022-1-TR01-KA220- VET-000088373), coordinated by Ankara University, Turkey.

This article provides a behind-the-scenes look at the development of this educational comic book, explaining our creative process, the methods we used to ensure accuracy and engagement, and how this case study is an essential educational tool for professionals across disciplines.



Why Thalidomide?

The Thalidomide disaster remains one of the most infamous drug scandals in history, responsible for tens of thousands of birth defects worldwide. This immensely tragic pharmaceutical disaster represents an invaluable learning opportunity about the importance of drug safety, regulatory oversight, and ethical responsibility in pharmaceutical development.

By transforming this case into a comic book, we sought to make the story of the catastrophe's personal and social impact more accessible and engaging for students and professionals who may not necessarily have a life science background. The visual medium allows us to explore the case's complexity in a way that captivates and informs the reader, fostering a deeper understanding of the potential consequences when pharmaceutical processes fail.


Our Approach to Public Health Storytelling

At Transmissible, we use narrative-driven educational tools to tackle public health challenges. For this project, we combined historical accuracy with compelling visual storytelling to bring the Thalidomide case to life. Our process revolved around three key pillars: interdisciplinary collaboration, visual engagement, and clear educational outcomes.

1. Interdisciplinary Collaboration

Developing this comic book was a collaborative effort between experts from various fields—medical specialists, pharmaceutical researchers, educators, and comic artists. The authoring team included:

Their insights greatly enhanced the quality and accuracy of this work, particularly from the perspective of those directly affected by the Thalidomide disaster.

2. Visual Engagement

The power of visual storytelling lies in its ability to connect emotionally with the audience. For this comic, we used visual metaphors and clear, impactful imagery to communicate the gravity of the Thalidomide disaster. Our team designed scenes that depicted the historical events and evoked the emotional toll on the victims, researchers, and society.

Each section of the pharmaceutical process — Discovery, Research, Development, Registration, Manufacturing, and Delivery — is represented with symbolic imagery to guide the reader through the complex stages that led to the catastrophe. This approach ensures that the story resonates visually and cognitively, making it easier for readers to grasp key lessons.

3. Clear Educational Outcomes

Our goal was not just to retell history but to offer a structured learning experience that could be integrated into pharmaceutical training programs. The comic is designed as a case study that educators can use to explore topics like:

  • Ethical responsibility in drug development
  • Regulatory failures and their consequences
  • The importance of rigorous clinical testing and post-market surveillance

The comic aligns with the learning objectives of our Erasmus+ training program, "From Molecule to Drug", which trains students in various disciplines involved in drug development.



Methodology: From Concept to Execution

Research and Story Development

Our process began with thorough research into the historical and scientific aspects of the Thalidomide case. We consulted historical records, academic articles, and firsthand accounts to ensure the narrative was factually accurate. The script was then developed in collaboration with public health experts to balance storytelling and education.

Visual Design

After finalizing the script, we carefully crafted the comic’s visual elements. We used a muted colour palette to reflect the nature of the subject while incorporating visual metaphors to represent abstract concepts like regulatory oversight, safety testing, and the consequences of negligence. Each scene was designed to visually engage the reader while providing enough detail to educate them on each phase of the pharmaceutical process.

Testing and Feedback

We are grateful for the input from Nuno S. Osório , Universidade do Minho on earlier drafts of this work. We acknowledge the support of Demet Cansaran-Duman (Ankara University), Aysegul Taylan Ozkan (TOBB University of Economics & Technology), and Kursat Derici (Kobay A.?.), who facilitated the testing of earlier draft versions. Feedback from pilot workshops with students in pharmaceutical studies showed that the comic was an effective educational tool, improving comprehension of pharmaceutical processes and oversight.


Results: What We Achieved

Our comic book has already had a positive impact in pilot training settings. Here are some key outcomes:

  • Improved Engagement: Students found the comic format engaging and memorable, particularly compared to traditional case studies.
  • Better Retention: Test results before and after the workshop showed an improvement in understanding complex pharmaceutical processes, especially in areas related to regulatory oversight and the consequences of skipping key safety steps.
  • Cross-Disciplinary Learning: The comic allowed students from different fields—pharmacology, medicine, and regulatory affairs—to appreciate the interdisciplinary nature of drug development.


What’s Next?

The Thalidomide Case: From Molecule to Catastrophe exemplifies Transmissible’s efforts to make public health storytelling a cornerstone of education. With the lessons learned from this project, we plan to develop more case studies in a visual format, covering other key moments in public health history.
