Two French Rafale Fighter Jets Collide and Crash During Return Flight from Germany
Two Dassault Rafale jets from the French Air Force’s Rafale Transformation Squadron (ETR) collided during a return flight from refueling in Germany, resulting in the deaths of a lieutenant and a captain, a student pilot, and his instructor. The collision occurred near Harmonville, Vosges, France, with wreckage found in a densely wooded area, complicating search efforts. One pilot from the Rafale C survived with injuries, while stormy conditions during the flight have been mentioned as a potential factor in the collision. ATSB Final Report: Stick Shaker Activation on REX SF34 Approach at Merimbula
The Australian Transport Safety Bureau (ATSB) released its final report on August 14, 2024, regarding the stick shaker activation incident involving a REX Regional Express Saab 340B at Merimbula, New South Wales, on June 8, 2023. The report identified that the incident occurred due to a combination of turbulent conditions, autopilot settings, and reduced engine power, leading to an undetected airspeed reduction below safe limits until the stall warning activated. Contributing factors included the activation of the ice speed system during the descent and a crew distraction related to windscreen wiper settings, prompting the ATSB to recommend improved crew awareness of autopilot modes and airspeed monitoring during approach phases in similar conditions. United B777-200 Diverts to Sacramento Due to Engine Trouble
On August 13, 2024, a United Airlines Boeing 777-200, registration N204UA, operating flight UA-507 from San Francisco to Rome, diverted to Sacramento International Airport, California, following reported engine issues with the PW4090 engine. The aircraft landed safely on runway 17L approximately 35 minutes after the crew reported the problem while cruising at FL300, about 70 miles northeast of Sacramento.
