Tools to succeed in Value Based Care

This past week, I had an opportunity to join a cohort organized by CareJourney by Arcadia on Value Based Care Analytics. The 2-day program was an excellent preview into how data can be a powerful instrument in helping us make business and clinical decisions that can change the status quo of the current care delivery and moving towards value based care.?I’d highly recommend it for all healthcare leaders.

Providers are handicapped due to lack of a true picture of patients, thanks to the siloed EHRs and health systems that are built around the transactional care we provide currently.?

While partnering with ACOs?gives the opportunity to deliver positive outcomes, independent practices are mostly limited to participating in Value-based care opportunities with just Medicare.

CareJourney by Arcadia and such analytics platforms?can truly empower independent practices in driving positive outcomes. Here is an opportunity for CareJourney to expand their platform to directly assist providers in:

• Identifying the specialists that are of high quality and cost efficient for our patients.
• Identifying the care gaps at the point of care
• Targeting high-risk and high-need patients
• Being more proactive in managing care coordination after acute events (ADTs)?
• Documenting and acting upon health inequities

We have ways to go in moving our health care system from volume to value.?We need the entire healthcare eco-system (including the main actor - the providers) be aligned in creating the right incentives, with a democratic access to patient data and insights that will motivate the stakeholders to make the right clinical?and business decisions.?
