No solution can be effective until the problem has been defined, the root cause(s) of that problem identified, and the conditions and actions create the chain of root causes.
Only then can the needed corrective and/or preventive actions be applied to increase the probability of success.
Then the solutions that implement those actions can be considered, and the needed effectiveness and performance of the application of the solutions can be evaluated.?
