TEETH IN A DAY – FULL MOUTH DENTAL IMPLANTS

Replacing a missing tooth/teeth and other supporting structures have been traditionally done by fabrication of complete and partial dentures (removable/fixed). However, for the last 40 years, dental implants have offered numerous advantages over these dentures. Improved comfort, aesthetically pleasing, durability and convenience are few of those advantages.

The endoosseous implants are inserted into the jaw bones over which a fixed or removable denture is fabricated. The retention of the implant into the bone comes from “OSSEOINTEGRATION”. This term refers to the bond between the bone and the implant surface. However, long treatment period still remains a huge disadvantage for endoosseous dental implant therapy.

Most of the patients that come to our clinic are usually apprehensive about the multiple surgical procedures involved with conventional implants and the year long wait for their final set of teeth.

Dr. Rajat Sachdeva has looked into this concern of his patients seriously and has effectively managed to alleviate these fears by introducing “SAME DAY IMPLANTS” OR “TEETH IN A DAY” OR “FIXED TEETH IN 3 DAYS”. WHAT ARE SAME DAY IMPLANTS?

The “same day dental implants” are called so because they are essentially placed on the same day that the tooth is extracted. The available literature has invariably cited high success rate of immediate implant placements (94%- 100% on an average). “Teeth in a day” concept is becoming increasingly popular among patients and dentists due to its multiple recognizable benefits.

HOW DO WE MAKE FIXED TEETH IN 3 DAYS?

We make this prospect a reality at Dr. Sachdeva’s dental clinic through years of experience by our renowned surgeon and implant specialist, Dr. Rajat Sachdeva. This procedure can be done for single teeth and even as a part of full mouth rehabilitation. Full mouth immediate implant procedure can be very challenging but with the state-of-the art technologies available at our clinic and surgical expertise of our doctors, it has become very simple.

Use of modern and exceptional implant systems like Nobel Active Impant systems from Nobel Biocare and Straumann? BLX implant systems, even the most difficult and challenging cases have shown good outcome.

Now, there are two different aspects to this - IMMEDIATE IMPLANT PLACEMENT AND IMMEDIATE IMPLANT LOADING.

While the immediate implant placement refers to placement of implant at the time of extraction, immediate implant loading is the loading of replacement teeth at the time of implant placement.

It is possible to perform both the procedures on the same day if the implant is stable enough. Or else, implant loading has to be done on a later date.

BENEFITS OF TEETH IN A DAY PROCEDURE:

The various advantages of “TEETH IN A DAY” or “SAME DAY TOOTH” concept are:-

? The surgeon can easily determine the appropriate parallelism and alignment of the implant using the extraction socket as a guide, relative to the adjacent and opposing residual arch.?

However, there are a few contraindications that needs to be considered before going ahead with this procedure.

If there is presence of an acute periapical inflammation associated with the tooth, immediate implant placement procedure is absolutely contraindicated. Further, the size and structure of the extraction socket site, no evidence of keratinized tissue, thin and delicate tissue, and absence of complete soft tissue closure over the extraction socket have been shown to prove unfavorable to immediate implant placement procedure.

It is generally advised that the surgeons avoid teeth with labial bony dehiscence or fenestration defects,any systemic factors or habits that may impair healing like smoking and cases of generalized periodontitis. Having said that, Dr. Sachdeva’s dental clinic has shown to achieve great results in some of the trickiest cases.

WHAT IS THE EXACT PROCEDURE THAT IS PERFORMED?

Unlike other clinical setups where the patient is usually unaware of the procedures that entail their treatment plan, we at Dr. Sachdeva’s dental clinic believe in educating our patients with all the details.

We try our level best to make the patient understand the procedure in simple and lay-man terms. Dr. Rajat and his team believes that this approach reduces the patient’s apprehension. It will not be wrong to say that we have seen the patients looking forward to their treatment once it has been explained to them.

SINGLE TOOTH IMMEDIATE IMPLANTS

In this procedure, we extract the compromised tooth and place the implant immediately. Thereafter, temporary restoration is placed so that the bone is preserved and so is the emergence profile. For the successful prognosis of this procedure, it is vital that the extraction be as atraumatic as possible. This helps to preserve the alveolar bone which is the foundation of a good outcome.

Following the extraction, implants are placed in the socket. The implants we use at our clinic are the modern, Swiss premium quality Straumann? BLX implant system and NobelActive implant systems by Nobel Biocare.

A temporary crown is fabricated and placed in the patient’s mouth the same day. This reduces the discomfort of the patient and does not hamper or interfere with the functions like talking and eating. It also is aesthetically pleasing, so that the patient doesn’t feel toothless. Subjected to satisfactory healing, permanent crown is fabricated after 3-4 months.

MULTIPLE TEETH / FULL MOUTH IMMEDIATE IMPLANTS

In a full mouth immediate implant procedure, 8 implants are placed on the upper arch and 6 implants on the lower arch. A full set of teeth are then fabricated upon these implants. We have a highly efficient lab at Dr. Sachdeva’s clinic where a good quality set of dentures are made in 3 days. The permanent set of teeth can be made of either Zirconia, Ceramic fused metal or metal reinforced acrylic dentures. However, in such cases, the follow up is of utmost importance. This follow up is usually started a week after the final insertion of dentures and is then regularly every 6 months.

We highly recommend that the patient practice good oral hygiene. Oral prophylaxis is advised biannually, followed by clinical and radiographic assessment of the implant status every year.

The procedural steps for placing multiple or full mouth implants are essentially the same as that of single tooth immediate implants. Although the concept of immediate implant placement looks simple and attractive, it requires meticulous planning and execution with fine precision.
