From nuisance to incident
When a system begins to exhibit early signs of malfunctioning the symptoms might be manifold. A most likely start is complaints from interacting personell. In practical use the system might just be unpleasant or even annoying. Responses seem unreliable, at times inconsistent or slow. Anecdotes of lost memories are circulating. Critical tests may be failing, go undone or stay overdue. Typically, it is just a matter of time until the incident occurs, a well reported outage with lasting consequences. Ideally, the issue is caught prior to the first incident, but often it is too late. With the first incident, the issue is logged into the organization's ticket system, prompting a formal assessment.
