“The collection of self-reported demographic data should be able to be populated from multiple sources into one common database and then shared systems and payers with the member's identifier like their SS#. Those sources of collection would need to be trusted sites from the perspective of the person. It can include websites for health plans, physician offices, hospitals, kiosks in government services centers, senior centers, churches, libraries, and community centers. Regarding social needs, the response to the inquiry can fluctuate over time, thus the data system should allow for updates and tracking changes/improvements.” - Adrienne Mims, MD MPH FAAFP AGSF
