Patient intake data protocols vary widely and impact patient data matching and retrieval of patient information. I received lots of feedback a few weeks ago related to a post about "Patient Matching". The waste that could be saved by eliminating or significantly reducing the failure to retrieve accurate patient information is substantial when considering the time and resources wasted today.
Based on the feedback from LinkedIn posts and direct messages, I expanded the fields on the Standardized Intake Protocols (SIP) form:
The second initiative is to move towards a digitized patient health history form. Paper forms may be keyed into the Electronic Health Records (EHR) system inaccurately or not keyed in at all. Instead placed on a paper chart or scanned into the EHR. Non-digitized data cannot be easily exchanged or retrieved. Therefore, more tests are often ordered to create a health history or time is wasted tracking down a print record.
Instead, a digitized patient health history form should be available in the patient portal for completion and updating and importing of their claims and clinical data with the result of an exchangeable, updated, and accurate health history.
