SMRT has a Moving Inspection Lab called the Multi-Function Vehicle or MFV back in 2014 !
With the recent axial box dropping out, Boogie wheels derail, then rail tracks crack. I am very surprised that SMRT has invested in this Rail Inspection Asset back in 2014 !

"Our Engineering Trains team has a moving inspection lab called the Multi-Function Vehicle or MFV.
This sophisticated vehicle is equipped with a Rail Flaw Detection (RFD) system that runs over the rails to detect internal cracks within the rails in real time. The system uses roller search units with ultrasonic transducers positioned in an array of angles for thorough test coverage during the rail inspection.
Our MFV is run at night, after the tracks are cleared of revenue service passenger trains, and can be found performing track inspections in tunnels and viaducts to keep the tracks safe for use.
SMRT engineers aboard the MFV analyse the ultrasonic data to ensure rails are not cracked. Operated by a crew of four, the MFV is also equipped with a Track Geometry Measurement (TGM) system which records and checks the geometry of the running rail and third rail which supplies electrical power to passenger trains.
The geometry of the track is important as it ensures the wheels of trains contact the rails at the prescribed angles for safe and efficient operations.
The MFV tips the scales at 32 tons. Speed self-propelled is 18km/h but it can move as fast as 40km/h when hauled by one or more locomotives.

So this opens the question ? Why this was not shared with the public that with this inspection equipment, why only at the late stage cracks were detected ????

The trains are equipped with Predictive Maintenance features and it was stated
"LTA said this allows the rail operator to “make more efficient use of maintenance resources to pre-emptively address potential faults before they affect commuter service”.
In addition, four trains are equipped with an Automatic Track Inspection (ATI) system to monitor running rails, track equipment and sleepers concurrently during passenger service.
“The ATI System supplements existing track inspection activities to identify rail and trackside components that require maintenance in a more timely and efficient manner,” said LTA."
Amazing that there is such capabilities in the East-West line already !
