Single incision laparoscopic colectomy again fails to demonstrate advantages

Yun et al recently published their series of 767 patients who underwent radical colectomy.  From this overall group they cohort matched 239 patients who underwent single incision laparoscopic (SIL) surgery with an equal number of patients who underwent conventional laparoscopic (CL) surgery.  After propensity score matching the only difference the authors could find was a significantly higher blood loss of 20cc. However, I cannot imagine that the 20 cc (20%) difference in blood loss even though statistically significant at P = 0.04, was in any way clinically significant.   The authors evaluated short term morbidity as well as long term oncologic outcomes.  The Kaplan Meier analysis of disease free survival revealed equivalent survival between the two groups.  The findings of the study were in exact accord with the results from another recent study which I recently described in a prior linkedin post.  Although this technique is feasible and appears as safe as does conventional laparoscopy, it has repeatedly failed to demonstrate any significant advantages.
