The International Statistical Institute adopted the first international classification of diseases in 1893. In 1898 the classification became known as the International List of Causes of Death and ultimately as the ICD.

Medicare/Medicaid started in 1965 along with their regulations. Not sure if overall health outcomes have changed much since 1955. Before infections that could not be cured, massive drug additions and more. ICD's went from a few pages to what we have today and is added to daily as we identify new pathological conditions that didn't exist before.

There were very few medical billing and payment regulation before 1965, which like ICD's went from a few pages to the size of the Library of Congress.

But the one thing that has not changed is the complaining about the cost.
