Odds are... your employer is in the health insurance business?
In a self-funded employer health plan, the employer is the actual insurer. The employer is at risk for the healthcare costs of their employees and family members, and uses the dollars they earn from selling products or services, to pay the cost of healthcare.
Most people don't realize that 60% of all employers in the U.S. operate their own self-funded health plan, and they cover over 100,000,000 lives (almost 1/3 of the population).
Because most employers are not in the insurance business, they sometimes contract with insurance carriers to run their health plan. FACT: 40% - 60% of the people carrying a major insurance company card, are not really insured by that insurance company. The insurer is only the administrator of their employer's health plan.
