Segmentectomy versus lobectomy in small-sized peripheral non-small-cell lung cancer with radiologically pure-solid appearance

The post-hoc analysis of the JCOG0802/WJOG4607L trial comparing segmentectomy and lobectomy for radiologically pure-solid non-small-cell lung cancer (NSCLC) revealed higher 5-year overall survival after segmentectomy (92.4% vs. 86.1%) with a median follow-up of 7.3 years. While locoregional recurrence was higher after segmentectomy, cause-specific deaths were similar. Subgroup analyses indicated better survival with segmentectomy in patients aged 70 or older and male patients, while younger patients (<70 years) and females had better relapse-free survival with lobectomy. This suggests age and sex may influence outcomes, urging further research for clinically relevant segmentectomy indications in pure-solid NSCLC.
