Going from reactive to proactive,to predictive risk management in an aviation SMS program takes many years and strategic planing on the part of an aviation safety manager.
We see aclear correlation between the rise of aviation SMS software and significant drops in aviation accidents.
Safety is not about the absence of something , but about the presence of an ability That enaables things to go right , across varying conditions
The War on Error: Human Error as a Strategic Risk Management Concern
It is necessary to reconize That human error will never be eliminated ,only moderated.
In order to combat human errors we need to change the conditions under which human work
The goal is not to eliminate the human factor because it is impossible,unless you eliminate the human .
The goal is to ensure an acceptably low level of human factor existence and minimize risk in the environment in which we operate.

Your toolbox may start out as an empty box, but it’s got room to grow, it’s scalable. You add some basic tools, and as time goes by you add more advanced ones to tackle more advanced projects. After awhile, each time you open it, you find the exact tool you need.
SMS is our toolbox for safety, and there’s no job more important.
Our toolbox has four large drawers (also known as pillars) that we’ll fill with tools to help make our operations safer for everyone. Once we’ve collected all our tools, we’ll be prepared to put it to use.

Employees in healthy safety cultures take time and energy to proactively identify and report potential hazards.
When employees see no activity or they receive no feadback, they will quickly become disillusioned with the aviation SMS and soon become disengaged employees.
From a past experience,i felt like i was wasting my time when i submitted safety concerns and disappeared in a black hole.
After a couple black hole experiences, i quit submitting.
In this case there was no transparency in the process,and no accountability to manage the process
