Why Report Incidents

Who is Involved

  1. Safety Manager: Oversees the reporting system.
  2. Other Managers: Encourage and support incident reporting.
  3. Safety Review Board (SRB) & Safety Action Group (SAG): Analyze reports and propose preventive actions.
  4. Maintenance Team Members: Play a significant role in reporting incidents.

The Reporting Process

  1. Immediate Reporting: Report unsafe conditions or incidents immediately using designated channels.
  2. Safety Department: Classifies, investigates, and analyzes the incident.
  3. Action Recommendations: Recommends actions to prevent future occurrences.
  4. Investigation Involvement: You may be involved in the investigation process if necessary.

Confidentiality and Protection

  1. Confidential Information: Reported information is kept confidential.
  2. No Negative Consequences: No adverse effects for reporting incidents.
  3. Just Culture: Promote a culture that values learning and improvement over assigning blame.

A few mistakes we committed and must be avoided : Ignoring Near Misses: Overlooking near misses prevents learning from potentially serious incidents before they happen Overcomplicating Incident Categorization: Too many categories or unclear classification can confuse users and lead to underreporting or misreporting of incidents. Blaming Culture: If the system is used to punish employees rather than improve safety, it will discourage open reporting.

It would be interesting to know what "designated channels" are used to immediately report unsafe conditions or incidents. There are various apps around that resolve the problem but for small organisations where cost is a issue what other solutions have been tried and found effective? e.g. emails, whatsapp group?
