Safety Eye Attachment for Handset Forms

On a recent project we were faced with fall protection challenges when working with handset forms. Unlike some newer formwork systems, such as Doka or Peri, which are manufactured with anchorage points engineered in, we were working with older Symons panels, stacked up to 18' high. We needed a solution for fall protection.

What we ended up with was a hybrid solution, including multiple scaffolds at varying heights, trying always to use the forms in front of the worker as a fall protection barrier. In some cases, though, such as during scaffold erection, we needed to tie off. 

One part of the solution was a Safety Eye attachment:

Along with these anchorage points (properly installed, of course), we utilized DBI/SALA NanoLok retractables with climbing hooks, as well as positioning belts and hooks.

As with any fall protection system, it may not have been perfect, and it all requires training and oversight, but we think it was all good steps in the right direction.

Does this take into consideration the engineering of a loaded form? Does the formwork with liquid head still provide adequate restraint without overloading the form in a fall scenario?
