On 18 June 2023, the OceanGate submersible the Titan descended to the Titanic shipwreck but lost contact 45 minutes later. The search began by the American and Canadian ships and planes but no trace was found until on 23 June 2023. The U.S. The Coast Guard announced a debris field of the Titan. Following that findings several videos published on the safety concern of the Titan. In business, this is a huge loss, including life and also possible loss from lawsuit. There are many risk assessment tools developed for the industry and even there are regulations on safety. How do these submarines disregard all these rules? Is there a theory of ignorance? There are 3 types of ignorance; factual ignorance due to absence of knowledge of the same facts, object ignorance which is acquaintance with some object and technical ignorance due to absence of knowledge of how to do something. The cause of ignorance is “ego” and the people obsessed with “ego” are self centered. It could also be related to personality types. These people could be the Type A personality, who are competitive, time is urgent, hostile and aggressive. There was a case in Malaysia, the new designed aircraft crashed on the 1st flight, which from the author review of the accident, there were no proper sequences of ground test before take off. One of the most important ground tests is high speed taxi and braking, which will determine the braking distance and balance field length, which is crucial for rejected take-off. Once these tests are completed then continue with the take-off test, which is based on a series of ground tests, there are already ideas of how the aircraft will climb.
