RUNNING - RELEASE THE PARKING BRAKE
Most people are walking or running with the parking break on. The big toe does not dorsiflex well while under load. It jams and inflames the 1st metatarsal joint creating bunions and deformation of the big toe. In athletics it is called "Turf Toe". The technical name for this is "Functional Hallux Limitus" or FHL. It is easily relieved by making a 2mm deep cutout in the forefoot of the foot orthotic, which allows the metatarsal head to drop and increases the ability of the big toe to dorsiflex while under load. You need 15 degrees of dorsiflexion to walk normal and 25 degrees to run well without jamming the big toe while under load.

9 年I use this technique fairly regularly for the athletes I treat with foot orthotics. I see it most often in basketball players. The simple reverse Morton's extension helps to displace pressure from the 1st MPJ and increases ROM to a certain degree. Chronic and increased inflammation, however, may necessitate surgical intervention to eliminate spurs The goal of conservative treatment through foot orthotics is to keep from undergoing surgery and improve quality of life.
