Rotating machine failures are avoidable. We should never stop asking the word 'why'!

Perhaps the effort required to avoid failures in rotating machines is too great, or the investment in time or money does not justify the returns or just sometimes we do not have the will.  But I believe that all rotating machine failures can be avoided.

It starts and end with asking why!  Why did the machine fail? it might be a seal or a bearing, a shaft, a gear, an impeller that was the reason for the failure.  The next question is 'why did the component fail?.  Why, why, why until the Root Cause Failure can be determined
