The answer is far from straightforward and depends significantly on the context, the complexity of the task, the level of accountability required, and the stakes involved. In healthcare, where human lives are at the center of these decisions, the implications are particularly profound. Let’s explore when healthcare AI should have a human in the loop and when it might be acceptable for AI systems to stand alone.

Human in the Loop: When Stakes Are High

In situations where patient safety is directly on the line—such as diagnostic support, treatment planning, or surgery assistance—AI must work in partnership with human experts. These are high-stakes environments where errors can have profound consequences for patient outcomes, including potential loss of life or severe harm. Human oversight ensures that critical nuances, patient histories, and ethical considerations are taken into account. For administrative functions where efficiency is prioritized, AI can often stand alone. The application of clinical decision making must remain with the clinician. They are the ones in relationship with the patient. AI can support and enhance appropriately, at strategically placed elements, but the final application rests with the clinician. They are the responsible, ethically caring provider.
