Returning to Practice: Reflecting on Medical Advances and Timeless Values
As I step back into the clinic after years away, I find myself overwhelmed by the rapid advancements in healthcare. The latest issue of the New England Journal of Medicine featured an article titled "Medical Artificial Intelligence and Human Values," which triggered a cascade of memories and reflections. These reflections took me back twenty years to a transformative trip to Tibet during my medical school years.
A Journey to Tibet: Rediscovering the Art of Medicine
In the heart of Lhasa, I visited a medical school and pharmacy, immersing myself in the rich tradition of Tibetan medicine. This journey was not just a geographical expedition but a profound exploration of the philosophy and practice of healing. The experience left an indelible mark on my understanding of what it means to be a physician.
This morning, as I dusted off the medical textbook I brought back from Tibet, I was reminded of the principles that underpin Tibetan medicine. This ancient practice, known as Sowa Rigpa, emphasizes a balance between body, mind, and spirit, underpinned by the physician’s compassion and intellectual rigor.
Tibetan Medicine: An Overview
Tibetan medicine is an intricate system with roots stretching back 2,500 years. It integrates elements from Indian Ayurvedic, Chinese, Persian, and indigenous Tibetan knowledge. At its core, Tibetan medicine revolves around the balance of three humors: rlung (wind), mkhris pa (fire), and bad kan (earth and water). Diseases arise when these humors are imbalanced.
Diagnosis and Treatment in Tibetan Medicine
Tibetan doctors employ a holistic approach to diagnosis, using observation, palpation, pulse analysis, and urine evaluation. Treatment involves a combination of dietary and lifestyle recommendations, herbal medicines, and external therapies such as acupuncture and moxibustion.
A unique aspect of Tibetan medicine is the deep ethical and compassionate relationship between doctor and patient. Physicians are expected to cultivate the Four Immeasurables: love, compassion, joy, and equanimity. This ethical framework ensures that the patient’s well-being is at the heart of every interaction.
Modern Medicine: Advances and Challenges
In contrast, modern medicine has seen remarkable advancements, particularly in the realm of medical technology and pharmaceuticals. The past year alone has brought us groundbreaking treatments such as CRISPR/Cas9 gene-editing therapy for sickle cell disease, new drugs for Alzheimer’s, and innovative gene therapies for muscular dystrophy. These advancements represent the pinnacle of scientific achievement, offering hope to millions.

Artificial Intelligence in Medicine
One of the most transformative developments is the integration of artificial intelligence (AI) into medical practice. AI has the potential to revolutionize diagnostics, treatment planning, and patient care. However, the NEJM article highlights a crucial aspect: the intersection of AI with human values. AI models, while powerful, are influenced by the biases and values of their creators and users. This calls for a careful and ethical approach to their deployment, ensuring that they reflect the diverse values and goals of patients.
Balancing Science and Art in Medicine
Reflecting on these advancements, I am struck by the contrast between the cutting-edge technology of modern medicine and the timeless wisdom of Tibetan medical practice. While science drives progress, the art of medicine lies in the compassionate care and ethical grounding that we provide to our patients.
In Tibetan medicine, the physician is not just a healer but a guide and a compassionate presence in the patient’s life. This is a lesson that modern medicine must remember. Despite our technological prowess, the core of our practice remains the human connection, the empathy, and the ethical commitment to our patients.
Moving Forward: Integrating Old and New
As I return to clinical practice, I am committed to integrating the wisdom of Tibetan medicine with the advancements of modern healthcare. This means embracing the latest technologies while maintaining the compassionate, patient-centered approach that defines true healing.
I believe that the future of medicine lies in this integration. By combining the precision of AI with the ethical and compassionate framework of traditional practices, we can offer a more holistic and personalized approach to healthcare. This journey is not just about healing the body but nurturing the mind and spirit, ensuring that we treat our patients with the utmost respect and care.
In this new chapter, I carry with me the lessons from Tibet, the advancements of modern science, and a renewed commitment to the art of medicine. Let us move forward, embracing both the science and the soul of our practice, to provide the best care for our patients.
