I received a call from a customer who was having trouble setting up MFA to use SMS, phone calls, or even Microsoft Authenticator through. https://aka.ms/MFASetup. The phone call or text message was not answered. When they scanned the QR code in the Authenticator App, the following error message appeared:
Activation failed. Make sure that push notifications are enabled on the phone and your Activation Code is not wrong, expired or formerly used
Solution Tip:
Sometimes, the user gets blocked from the MFA in the Azure Portal. So we have to unblock that user from the Azure Portal. You will see the step by step actions below. To get him unblocked, go to Azure Portal -> Azure Active Directory -> ( -> Security) MFA
Click Block/unblock users and click unblock on the blocked user. They can now successfully setup the Authenticator App.

10 个月This helped me just now too, had a user who kept having their Authenticator app prompts apparently be denied without them doing anything. They were listed in Azure as a blocked user and needed unblocking for MFA to work again. I think if a prompt is ever denied even accidentally it may block the user completely. Thanks for the help! ??
Thank you so much for this article, this worked for us
