Report: The Rise of Self-Proclaimed Gurus in Thyroid Ablation and the Role of Aggressive Manufacturers
The burgeoning field of thyroid ablation, especially Radiofrequency Ablation (RFA) and Microwave Ablation (MWA), has seen a troubling trend where medical device manufacturers, particularly from South Korea and China, are leveraging fast-track training programs to promote their devices.
These programs are often endorsed by self-proclaimed "gurus" who lack the necessary experience and expertise.
This approach prioritizes sales over patient safety, creating significant risks for patients, especially when practitioners are not adequately trained in the critical skill of ultrasound guidance.
The Importance of Ultrasound Expertise in Thyroid Ablation
Ultrasound guidance is a cornerstone of successful thyroid ablation procedures. The ability to precisely navigate and target thyroid tissue while avoiding critical structures like nerves and blood vessels requires not only technical skill but also years of experience.
As noted by Dr. Jagdish Dhingra , mastering ultrasound techniques is essential for performing minimally invasive procedures like thyroid ablation safely. However, not all doctors possess the necessary ultrasound skills, and gaining proficiency requires extensive training and practice over several years.
Unfortunately, many of the fast-track training sessions sponsored by manufacturers are inadequate in providing this essential experience. These programs often emphasize the ease of use of their devices rather than the complexities involved in safely performing the procedure. These courses often overlook patient selection as highlighted by Dr. Giovanni Mauri below:
?? The result is a new wave of practitioners who may be able to operate the device but lack the nuanced understanding of ultrasound anatomy and technique required to minimize patient risk.
The Risks of Inadequate Training When a surgeon or ablation practitioner is not highly skilled in ultrasound, the potential for complications increases significantly.
Misplaced ablation, unintended damage to surrounding tissues, and incomplete treatment of the thyroid nodule are all risks associated with inadequate ultrasound proficiency. These complications can lead to further medical interventions, additional costs, and significant distress for patients; as discussed by Dr. Mauri in the below video:
According to experts like Dr. Emad Kandil , who has treated over 1,000 thyroid ablation cases, including some of the most complex thyroid cancer ablations, the importance of ultrasound expertise cannot be overstated.
Dr. Kandil has noted a rise in patients requiring corrective treatment after undergoing poorly performed ablations by less experienced practitioners. This surge underscores the dangers of allowing practitioners to perform these procedures without the necessary ultrasound skills, which cannot be adequately developed through fast-track, manufacturer-sponsored training programs.
Manufacturer Exploitation of the Market
?? Korean and Chinese manufacturers have aggressively entered the thyroid ablation market in all corners of the world, using these fast-track training sessions as a tool to quickly expand their market share.
By focusing on the ease of use of their devices, these manufacturers downplay the complexities and risks associated with thyroid ablation, misleading practitioners into believing they can achieve competency with minimal training.

As a result, these companies prioritize device sales over patient safety, leveraging their relationships with self-proclaimed gurus to endorse these inadequate training programs. This trend not only jeopardizes patient outcomes but also erodes the trust in thyroid ablation as a safe and effective treatment option.
The Call for Stricter Oversight and Training Standards
To protect patient safety and ensure the long-term viability of thyroid ablation, there is an urgent need for stricter oversight and more rigorous training standards. Professional medical associations and regulatory bodies must take a stand against the commercial exploitation of training and ensure that practitioners are thoroughly trained in both the technical and clinical aspects of thyroid ablation, with a particular emphasis on ultrasound proficiency.
Medical journals and professional publications, such as The Lancet, have long advocated for more stringent regulations on medical device marketing and the need for comprehensive training.
The responsibility lies with both the medical community and the manufacturers to ensure that the adoption of new technologies is guided by patient safety rather than commercial interests.
Final Thoughts
The rise of self-proclaimed gurus and the aggressive marketing tactics of thyroid ablation device manufacturers pose significant risks to patient safety. Without the necessary expertise in ultrasound guidance, practitioners are ill-equipped to perform these complex procedures, leading to suboptimal outcomes and increased patient harm. It is imperative that the medical community takes action to address these issues, ensuring that training programs are led by truly experienced professionals and that patient safety remains the top priority.
For the future of thyroid ablation to be secure, the medical community must demand higher standards of training and resist the commercial pressures exerted by manufacturers.

7 个月interesting viewpoint with a very sensationalized writing style and tone. I agree with the need for credentialing and oversight in development of new technologies. I did not see any of your "experts" in the article mention checking for the vascular flow of the lesions using color or power doppler, use of US contrast injections to determine residual viability, measurement of post-ablation echogenicity changes, and suggestions for follow-up. Also not mentioned were attention to the environment in which the procedure is being performed, having necessary life saving equipment readily available, airway management equipment readily available, sedation protocols and expertise , anesthesia expertise. Let's have a practical and real-world discussion to advocate for the safety of our patients and move away from sensationalized journalism.
Hallelujah!!!!!! Shout this from the Mountain Tops!!!!! Profits over Patients is Never acceptable!!!!
