REACH Model Shows Modest Growth, Enabler Participation, and HNP Expansion

Last week, CMMI released statistics about the new cohort of REACH ACO applicants, identifying which have been provisionally accepted to the program.?Some interesting observations:

  • Overall acceptance rate: Only ~47% of the applicants were accepted (128 vs 271), indicating that CMMI was quite selective.
  • By model type: Looking across model types, New Entrant (NE) ACOs and High Needs Population (HNP) models had a much lower acceptance rate than Standard ACOs, likely tied to additional requirements that needed to be met.
  • HNP growth: Despite having a low acceptance rate, there are many more potential HNP ACOs (22) than current HNP DCEs (8).
  • Program growth: Since REACH is intended to replace CMMI’s GPDC model and the 99 active GPDC DCEs are eligible to go into REACH, the program could more than double in size for PY 2023, growing to as high as 227 ACOs. But the actual number will likely be lower. First, not all of the DCEs will want to go into REACH or be able to meet all of the requirements of the program. Second, some of the provisionally accepted REACH ACOs could drop out if they have multiple applications for participation in different geographies or are still deciding between MSSP and REACH programs.
  • MSO / Enablers: Despite wanting the REACH program to attract more provider-led applicants, 42% of the applicants self-reported as being “MSO (Managed Services Organization) / Convener”, which is about on par with the current GPDC cohort. This number is likely higher if venture-backed multi-state ACOs are included.
  • By organization type: The acceptance rate varied quite a lot by organization type, from about 25% to 60%.

  • July 2022: Participation Agreement for optional Implementation Period (IP3) due
  • August 1, 2022: Optional Implementation Period (IP3) starts
  • August 4, 2022: Final provider list additions due
  • September 9, 2022: Final provider list drops and option selections due

Very interesting, especially the lower percentage of ACO applicants accepted in to the program. Encouraged to see the increase in HN ACOs over HN DCEs that will be participating.
