Quality and Productivity not dependent on Hours Spent

In our IT industry, we often focus on Number of hours spent in office as primary and the work done in a day as secondary . I have seen many employees going through only one question as first Q by seniors - Why not 8 or 9 hours? Why less than that? and next is -Show me what have you done whole day, or provide task/activity list did in a whole day.
We should understand the Quality of work done in what time. I am not saying spend only 2 hours and go home, this should not be practiced and shows very casual attitude. But as we have imposed 8/9/9+ hours on our employees, then we should query or dig around the quality and productivity. Is it degrading or powering? I have seen so many emotions and tensions arises in organisations, where an employee hardly talk about it with managers/seniors/HRs, it may be due to reasons like-What if it affects my appraisal. What if they Fire me? What if I am not able to learn and progress, may be I am seeking help.
I have come across with expecting mothers in org -They are simply sitting in office to complete those mandatory hours. Is it so bad in our industry to deal these kind of situation ? We have Women council, Women networks in offices, but do the really help - I have seen these council managers suggesting them -Please ask your manager if they permit ? Please at least complete your mandate hours.
Then why these Councils are made?
Not only this, we have so many stories and experiences around us, that if we are working qualitatively in acceptable hours, management is happy -But what if that person is experiencing some trauma at home/personal life /health issues -Do we really care asking or even conducting such meetings/discussions with them?
If we are imposing or tightening the policies, in parallel we should also plan and circulate those motivating policies and activities. So that these kind of mandate policies do not hamper work and its deadline, health, quality and productivity.
Increase in mandate hours or counting those hours -from 8/9 hours to don't know how long in office --Will this really help increasing quality and productivity? or will it boost more Tensions/Stress/burdens/fear/health degradation/less focus/less personal and family time?
