As a Physician any decision I make with regards to safeguarding the health of my patients and the public, in order to be medically correct, has to be made without prejudice or influence by any person or group and must be based solely on medical data alone and without any economic or political motives. This Pandemic has forced physicians such as myself to use my formal education and training along with empirical?knowledge of diagnosing and treating medical illnesses that I have gained from years of actively practicing medicine to make decisions throughout this Pandemic pertaining to the safety and well being of my patients and the public. My Common sense always takes precedent when it comes to making a final decision with certainty to the best of my ability to believe that I have made the correct decision when treating a patient.?????????????????????????????????????Common sense led me to use deductive reasoning to decide that for the protection of my patients and the public, I concluded that a?large public gathering would cause potential harm to the safety and well being to the public if I held the Annual Outdoor Christmas Celebration last week that was planned to take place outside my office (?https://twitter.com/compose/tweet/media?). Common sense told me that 1-this present December 2021 spike in Covid 19 cases was infecting people at an accelerated pace we've not seen in the past during this Pandemic and 2-this virus has mutated and is now infecting people who have been fully vaccinated, having received 2 doses of a vaccine and a booster. We're seeing a dramatic increase in what we've come to call "breakthrough cases", along with this virus infecting the unvaccinated or under vaccinated as well in unprecedented numbers, with no identifiable pattern in terms of age groups being infected and without any specific pattern with regards to the preferred host of this virus causing harm predominantly in individuals with comorbidities or less healthy immune systems such as the elderly and immunocompromised as we witnessed before. We can only assume that this "smart virus" has evolved (survival of the fittest) into mutations that have developed "different defense mechanisms" developed by successfully avoiding destruction by the?three or four vaccines. We have to assume that these mutations (variants) are combining with each other creating the potential for the emergence of Covid 19 Variants that have outsmarted the defense systems we have created to fight this virus. We have used Vaccines as weapons to kill this virus that obviously need rethinking in terms of the false sense of security our healthcare officials feel are sufficient enough (proof of Vaccination Status) to protect us and prevent another round of serious harm in terms of again, this winter, experiencing, once again, alarming numbers in terms of the morbidity and mortality rates we've seen this virus cause which soon will is soon be be close to a million deaths in the United States alone.?

Clearly, these Vaccines which with our health officials have focused on with sincere intentions to protect us and indeed have been successful to a large degree in doing so, are not enough. We should now focus on the present dramatic surge in "breakthrough Covid 19 Cases" of individuals not just testing positive for this virus, but a drastic alarming increase of Covid 19 illnesses presently infecting fully vaccinated individuals.? PLEASE NOTE HOWEVER THAT IT IS THE UNVACCINATED WHO ARE SUFFERING MORE SEVERE ILLNESS PRESENTLY WITH MANY REQUIRING HOSPITALIZATION WITH INCREASED RISK OF DEATH. THOSE WHO HAVE BEEN VACCINATED ARE EXPERIENCING MUCH LESS SEVERE RESPIRATORY ILLNESS AND ARE DOING WELL WITH TREATMENT. IN EITHER CASE, THE MOST IMPORTANT FACTOR IN ACHEIVING COMPLETE RECOVERY WHEN SYMPTOMATICALLY INFECTED WITH COVID IS TO CONTACT YOUR HEALTHCAE PROVIDER IMMEDIATELY WHO MAYWANT TO START YOU ON A REGIMEN OF PREDNISONE, A PROPHYLACTIC ANTIBIOTIC AND BRONCHODILATOR INHALER, A REIMEN I SEE MORE AND MORE PROVIDERS PRESCRIBING, A REGIMEN I HAVE USED THAT HAS PROVED TO BE SUCCESSFUL SINCE THE START OF THIS PANDEMIC.

I've stated from the start of this Pandemic that we must put more thought into treating this virus and now with urgency, perhaps even prophylactically, in any individual testing positive for Covid 19. I advocated for using steroids and a prophylactic antibiotic from the start stating that by chance, I gained knowledge empirically) as many of my patients live in what was declared the first "Covid 19 Epicenter" in the U.S., the first "Containment Zone" in the U.S.. I reported to our Health Officials that for a full two months before we ever heard of this NEW ("novel") VIRUS, and before tests to detect the virus were available,?I took notice of the fact that I was treating an extraordinary amount of patients with an acute respiratory syndrome that had a defined pattern that was different than any previous seasonal flu-like viruses I was familiar with and had seen over many years in the past. As many of my patients remember, all day long for 2 months before testing was available, I stated to my patients that this was a NEW and DIFFERENT type of virus this year, adding and pre-informing my patients "I may have to give you two rounds of steroids to treat this". I treated hundreds of patients without anyone getting Covid Pneumonia nor anyone requiring hospitalization, all those who were compliant with treatment. I'm saddened to say that I lost two patients who were noncompliant, one being pre-maturely placed on a vent as parameters had not yet been changed, realizing that vents were to be saved as a last ditch effort to treat Covid 19 Pneumonias as vents inhibited ones ability to expel this virus by inhibiting ones natural immune defenses such as cough to expel this virus from ones respiratory system. My other patient sadly waited too long before seeking treatment, refusing treatment actually. Both these patients weigh heavily on my mind to this day.??

So it's time we give more though to treating this virus, if not prophylactically, at least develop a protocol to treat Covid 19 symptomatic individuals early on and as soon as respiratory symptoms appear, realizing from the current surge in breakthrough cases (increasing #s of individuals becoming infected now after being fully vaccinated. Although the vaccinated population of breakthrough cases are presently experiencing less severe illness than the unvaccinated population, there is no guarantee that this will not change.?

I can't emphasize enough my concern over the need to direct our efforts to developing an?OUTPATIENT TREATMENT "PROTOCOL" for treating Covid 19 respiratory infections early on, to prevent progression of and replication of this NOVEL CORONA VIRUS AND IT'S VARIANTS. It's time we realize from what we are presently witnessing during this December 2021 Covid 19 surge that we can't rely on proof of vaccination status alone to protect the public in certain situations and I would hope that our Health Officials who are deciding on protecting the public and all of those residing in Manhattan, that if we are to allow a huge crowd to gather in Times Square this New Year's Eve, we have to either cancel as vaccination status alone is not protective enough, or we have to implement and strategize over what other protective mechanisms we can implement to protect the public and especially residents of NYC who already are showing the highest numbers of people testing Covid +. I know that many of you reading my posts and articles are public health officials.
