India. A promising therapy for Covid 19: Molecular hydrogen (H2) inhalation.

One of the COVID-19 treatments involves inhaling a mixture of gaseous hydrogen and oxygen, obtaining better results than with oxygen alone.  

Molecular Hydrogen, being the smallest molecule, is highly diffusible and is able to permeate through the blood/brain barrier, lipid membranes, cytosolic fluid, and into the cellular organelles. In particular, the small molecular properties of hydrogen ensure that it quickly reaches the alveoli, which suggests a unique advantage for lung diseases. 

Molecular hydrogen can selectively react with hydroxyl radicals (?OH) and peroxynitrite (ONOO-), the main contributors of oxidative stress without disturbing metabolic redox reactions. 

Molecular hydrogen can significantly down regulate expressions of inflammation-related genes, found to remove ROS, increase the action of superoxide dismutase (SOD), can effectively suppress the OXIDATIVE STRESS and CYTOKINE STORM when getting infected with severe viruses. 

Since persistent tissue damage, MODS (Multiple Organ dysfunction syndrome) and high mortality rate are highly associated with OXIDATIVE STRESS AND CYTOKINE STORM induced by Covid 19, molecular hydrogen can significantly reduce oxidative stress and RESTRAIN EXCESSIVE PRODUCTION OF CYTOKINES. Molecular hydrogen It can also help resist immune-led apoptosis (or programmed cell death) among cells of the body. MAY REDUCE THE DEMAND FOR VENTILATORS. 

Clinical studies have shown Molecular hydrogen protects lung from hypoxia/re-oxygenation injury by reducing hydroxyl radical production and inhibiting inflammatory responses, ameliorates ventilator-induced lung injury and attenuated pulmonary fibrosis. 

Early molecular hydrogen inhalation may promote sputum dilution, improve small airway resistance, and relieve dyspnea. Furthermore, breathing H2-O2 might reduce the inspiratory effort in patients with acute severe tracheal stenosis and can be used safely for this purpose. 

One of the clinical papers observe that H2 should be used in combination with other treatments such as anakinra (an IL-1 receptor antagonist) or tocilizumab (an IL-6 inhibitor), or in conjunction with O2, considering molecular hydrogen (H2) is likely to be a complimentary, restorative molecule. Equally, since Molecular hydrogen (H2) is able to help modulate and regulate cytokines (i.e., production and consequence), it would be fitting to be used in combination with such targeted immunomodulatory drugs. 

European respiratory Journal: 

In light of the significantly decreased airway resistance and safety, inhalation of hydrogen and oxygen mixed gas which is generated through water electrolysis has been applied in clinical practice. Hydrogen/oxygen mixed gas inhalation resulted in major amelioration of dyspnea in most patients with Covid-19 in a pilot investigation, and a has therefore been endorsed by the latest Recommendation for the Diagnosis and Management of Covid-19 document.

Dr. Zhong Nanshan, the epidemiologist who discovered the SARS coronavirus in 2003, recently recommended the inhalation of molecular hydrogen for COVID-19 patients based on the preliminary clinical data. Hydrogen treatment resulted in significant amelioration of dyspnea in most patients with COVID-19 in the pilot investigation. Correspondingly, hydrogen inhalation is included as a recommendation by the National Health Commission of The People’s Republic of China’s protocol for COVID-19. 

Researchers from the Department of Critical Care Medicine, Sichuan Provincial People's Hospital, Chengdu in China, have explored the possibility of using hydrogen therapy on severely ill COVID-19 patients. The team published their study's findings, titled "Hydrogen: A Potential New Adjuvant Therapy for COVID-19 Patients," in the latest issue of the journal Frontiers in Pharmacology.

WHO recommends molecular hydrogen as an adjunct line of therapy.  Two multicentre randomized controlled trials (RCTs) with inhalational hydrogen for COVID- 19 are listed in the World Health Organization

At present, patients with coronavirus-19 pneumonia are treated with high flow pure oxygen (without adding hydrogen), although the effect of O2 when associated with hydrogen may give better results. The production of mucus in these patients reduces the absorption of oxygen, while with a mixture of oxygen and hydrogen the bronchioles and the alveoli of the lungs are further expanded, optimizing the absorption of oxygen . Molecular hydrogen is used as a catalyst to accelerate the binding of hemoglobin with oxygen. 

This approach may allow clinicians to effectively administer both the oxygen needed to ensure optimal gas exchange within the lungs while also limiting the likelihood of hyperoxia-induced ROS/RNS accumulation via the cytoprotective mechanism.

The early use of molecular hydrogen (h2)  might mitigate the destruction caused by the cytokine storm associated with COVID-19, reducing the incidence of critically ill patients. In view of the outbreak, transmission, and widespread nature of COVID 19 virus, and the global issues caused by new variation pandemics threats, molecular hydrogen medicine may give us more hope for greater survival and fewer human morbidity and mortality rates.
