PINHOLE SURGICAL TECHNIQUE - EVERYTHING YOU NEED TO KNOW

“Perpetual preservation of what remains is more important than the meticulous replacement of what is missing” has always been the goal of dentistry and our dentists at Dr. Sachdeva’s dental clinic strictly adhere to it. Dr. Rajat Sachdeva has always strived to learn the latest and newest techniques in the field of dentistry with the primary motto of giving his patients a comfortable, high-quality dental experience.

The pinhole surgical technique is a novel treatment modality that has been introduced and is becoming increasingly popular among our patients. It has almost replaced gum grafting in treating gingival recession due to its less invasive approach. Before we dive into what pinhole surgical technique has in store for our patients, let us first address the issue of gingival or gum recession.

WHAT IS GINGIVAL RECESSION?

Gingival recession or “receding gums” is one of the most common clinical situation that we see in our daily practice. To put it simply, it is nothing but the exposure of roots along the gum line.

WHY SHOULD IT BE A PROBLEM?

Well, the root is supposed to be covered by the gums as it does not contain the hard, resistant enamel. So in case of exposure, it leads to sensitivity and other problems like loose teeth. Gingival recession can be caused by a number of reasons including mainly over-brushing, misaligned teeth and poor oral hygiene. While there are a number of ways you can correct this like by using a non-surgical option such as fluoride-desensitizing toothpaste to an invasive surgical approach like gum grafting. Gum grafting is a traditional surgical procedure wherein tissue is taken from somewhere else (in your mouth) and patched on the receding gums. It is definitely an exhaustive surgical procedure and resultantly takes longer to heal. Pinhole surgical technique has been largely replacing the traditional gum grafting. There are numerous benefits that the former has to offer. Before we move on to the advantages, let us familiarize ourselves with the pinhole surgical technique.

WHAT IS PINHOLE SURGICAL TECHNIQUE?

The pinhole surgical technique helps to treat receding gums quickly, safely and effectively. It was developed and patented by John Chao D.D.S and our dentist at Dr. Rajat Sachdeva’s dental clinic have been practicing the same for quite some time now. In this procedure, small “pinholes” are made in the gums above the affected area using a needle and then the gums are moved in a coronal direction. Small pellets of collagen fibers are inserted through this hole to help in stabilization and regeneration. Since no incisions or cuts are made in this procedure, there is no need for sutures.

PROCEDURE :-

After the initial consultation with your dentist, he will determine whether you are healthy enough to undergo this procedure. If he agrees, your procedure will start by giving a local anesthesia. It will numb your tissues so that the entire procedure is painless and comfortable.

The dentist will then clean the affected area and make small holes in a place above your receding gums. This pinhole is done by a small, hypodermic needle of 16 gauge. After that, using specially designed instruments, the gum tissue is loosened and moved in a coronal (towards the crown of the tooth) direction so as to cover the receding gums. This is done without making any cuts in the gums or damaging the surrounding tissues.

The gums are stabilized in the new position using small pellets of bioresorbable collagen membranes. These pellets are inserted through the pinholes and are tucked onto the root surface. This will help in the regeneration of new, healthy tissue. This technique has a unique feature that it does not require any sutures or stitches to stabilize the surgically moved tissue.

BENEFITS

The novel pinhole surgical technique offers the following advantages over the traditional gum grafting :-

1. Non-invasive: Since pinhole surgery doesn’t require the use of scalpel or blade to make cuts on the tissue, there is no collateral damage to surrounding tissues.

2. Quick recovery time: As the surgery is less traumatic, the healing is way better and the patients have reduced post-operative complications like pain and swelling.

3. No need for grafts: Unlike gum grafting where a tissue graft is required to cover the receding gums, pinhole surgery doesn’t require harvesting of grafts. Thus, it reduces unnecessary trauma to a healthy site.

4. Instant improvement: With pinhole surgical technique, the patient will almost instantly notice the new changed smile and coverage of the gums.

5. No need for sutures

AM I A GOOD CANDIDATE FOR THIS TECHNIQUE?

Pinhole surgical technique targets and treats gingival recession. If you are experiencing receding gums due to various reasons like incorrect brushing, genetics, smoking, poor oral hygiene, or bruxism (teeth grinding), this technique might be a good option to make your gums contoured again. This technique will not only correct your gums to a natural shape but will also improve your smile. Having said that, it is important that your gums are in healthy condition before your dentist can perform this procedure. This means that there should be no inflammation in your gums. If there is, our dentist will treat beforehand.

HOW FAST I CAN RECOVER?

Since it is a non-invasive procedure, the recovery time is relatively quick. Any pain or swelling should completely subside within a week. We will give you all the necessary post-operative instructions so that you can be fully recovered within no time. The collagen membrane is bioresorbable and is slowly resorbed in about 3-4 months. Maintaining good oral hygiene for a healthy healing period is of utmost importance.
